This waiver covers participation in our high school ministry events throughout the year, including activities such as the Salmon la Sac camping trip, retreats, service projects, and other ministry events.
Completing the waiver now helps ensure your teen is ready to participate in upcoming activities without any delays. Thank you for taking a few minutes to fill it out!

Medical Permissions (Limited): As a condition of attending Salmon La Sac Camping Trip, I grant permission in the event of an emergency or an accident for emergency medical care to be administered to my child within the facility and/or during or after the transportation to a hospital or doctor for emergency medical care. I further understand that it is not the responsibility of the Seattle Church of Christ to attempt to reach my child's emergency contacts, and that I remain responsible for my child's medical expenses.
Medical Information: Pertinent information about allergies or health problems, present medication and dosage which your child may be taking and any other information that will enable the adult leaders to obtain safe medical treatment for your teen must be included on the below Student Medication from.
*Please note, chaperones will NOT be collecting or administering medications for this event. Your child will be responsible for themselves.
Salmon La Sac Camping Trip participants agree to the following:
To be considerate, sensitive, and respectful of other attendees, adults, and the surrounding property through behavior, language, and dress. This includes obeying direct requests from adult supervisors.
- All participants are subject to a bag check if a supervising adult feels it is necessary. Teen may be present for the bag check, which would be conducted privately by an adult of the same gender.
To refrain from inappropriate touching, talking, texting, and the like, including anything sexual in nature or flirting.
To refrain from actions that could result in injury and/or property damage.
To adhere to the stated curfew, including back to their house or lights out.
To attend all scheduled activities, arriving promptly, and staying for the entire event.
Report any problems to an adult leader.
Travel in groups of three or more.
Always tell an adult leader if they will be leaving the main event area.
Salmon La Sac Camping Trip participants agree to not:
Possess weapons or fireworks of any kind.
Purchase, possess, consume, or distribute alcohol, illegal drugs, or tobacco products.
Engage in any form of sexual activity, flirting, etc.
Use profanity or degrading language of any kind.
Visit or gather in the sleeping areas of the opposite gender.
Only jumping in chaperone approved locations.
***ALL TEENS (including graduated seniors or teens who are 18) are NOT allowed to leave the campsite without the Bridges or Ramazanis even if they drove themselves until Sunday after check out or if their parent let us know beforehand that they would need to leave early. We will make store runs/provide everything they need!
Failure to adhere to this code of conduct may result in reprimand, removal of privileges, or dismissal from the event, at the discretion of the adult leaders.
CANCELLATIONS: You may cancel by email to Becky Meadows at beckym@seattlecoc.org before August 23rd for a refund minus the processing fees. There will be NO REFUNDS for cancellations received after August 23rd, 2026.
